At J&R Swanberg Community Residence our goals are to provide hope and recovery for people with psychiatric disabilities, and to stabilize symptoms allowing the resident to reach and maintain highest level of independence possible.

Toward this goal, each resident works with staff to form their individual service plan. Residents are encouraged to participate in creating their own service plan and choose objectives that focus on their individual strengths and weaknesses. These goals are then evaluated periodically with the resident in order to provide positive reinforcement for each to realize their chosen goals. This process of designing and revising the individualized program is to assist the resident in obtaining and maintaining a psychiatric rehabilitation goal and making progress which is measurable toward that goal.

Transportation to day programming, medical, and psychiatric appointments is available seven days a week.  Residents are encouraged to attend a day program such as day care, psychosocial club activities or sheltered workshop. The Psychiatrist and Service Coordinator assist the resident in choosing a program specific to their individual needs.  

Our staff provides a structured, supervised environment where health, medication management, and nutritional issues are of utmost importance.  Staff is experienced in dealing with issues related to mental health such as the difficulties of schizophrenia, depression, anxiety and other mental health issues.  Staff and resident work together as a team with psychiatrist, therapist, case manager, and family members to develop a treatment plan based the individual resident's needs. 

All medications are recorded and stored in a locked medication room and are either administered to the resident or self-administered under supervision according to the resident's level of capability. All controlled drug substances are stored under double lock as mandated by law. For any resident requiring laboratory work, the Centrex Laboratory Phlebotomist comes to the home on a weekly basis or immediately by Doctor order.
